Including a crisis plan in your person-centered plan is an essential part of ensuring your well-being and safety. At Herbert Reid Home, we are committed to helping you prepare for unexpected situations by working closely with you to create a personalized plan that both prevents crises when possible and supports you effectively if a crisis does occur.
Your crisis plan will outline your preferences, needs, and the actions you’d like others to take in the event of a crisis. This may include:
Who to contact—such as trusted friends, family members, or health professionals
Emergency contact numbers
Preferred medications or treatment approaches
Instructions for the care of children, pets, or other responsibilities
Arrangements for paying essential bills or managing finances during a crisis
Herbert Reid Home will also provide you with the name and contact information of the staff member or team to reach out to within our agency should you find yourself in crisis.
By proactively developing a crisis plan, you and your support team will be better prepared to respond quickly and compassionately, ensuring that your voice and preferences are respected even in challenging times.
