Alternative Family Living (AFL)

Alternative Family Living (AFL) homes have their own identities, life goals, and varied levels of needed support. Supported by Easterseals PORT Health, individuals also work on their daily and independent living skills while being integrated into the household and the community. Depending on the person, they might attend a day program or receive other services in addition to the assistance provided by AFL hosts.

For many years, we have successfully matched hosts and individuals to ensure the right fit. In addition, our AFL providers undergo extensive training and are supervised by our team to ensure they continue offering a safe, stable, and caring home.

Alternative Family Living program goals:

• Increase or maintain the individual’s life skills
• Receive the supervision needed
• Maximize their self-sufficiency
• Increase their self-determination
• Enjoy opportunities to become part of the community – like volunteering, working, and participating in social outings with friends.

The 1915i services

The 1915i services are for members who want support to live in their home communities. These services are non-medical behavioral health services, such as supported employment. They are provided at a member’s home or residence or community, and are not for members living in an institution.

Please note: 1915(i) services replace many of the previous 1915 (b)(3) services. The 1915(b)(3) services are not longer available.

The 1915(i) services include:

  • Community Living and Supports
  • Individual and Transitional Support  
  • Individual Placement & Support for Mental Health & Substance Use
  • Respite  
  • Supported Employment for Member’s w/ Intellectual/ Developmental Disabilities (IDD) or Traumatic Brain Injury (TBI) 

Get help with daily tasks at home, like bathing, dressing, cooking, budgeting and more. A trained worker will help you one-on-one based on your individual needs.
